Initial Visit

At your initial appointment we ask that you please bring along the following:

-insurance information (policy holder's name and date of birth, insurance company name, policy number and certificate number)

-a list of all medications that you may be taking. (prescriptions, non-prescriptions, vitamins and herbals)

Adult

An adult initial visit will take approximately 1 (one) hour.  At this appointment a complete medical and dental history is taken, any necessary x-rays will be taken and a comprehensive exam will be performed.  A comprehensive exam consists of an examination of the hard and soft tissues to check for any abnormalities as well as to check for cavities.  This is done to assess your dental needs and prepare a customized treatment plan for you.

Child

A child's initial visit will take between 40-70 minutes depending on the child's age.  This appointment includes a hygiene appointment.
